Blocks in a Box™ Order Form

An order placed with this form will be sent to the customer's shipping address. If you wish to have the set of Blocks in a Box™ that you order sent directly to the child's home, use our Gift Order Form.

Holiday Delivery Notice! Any order placed after December 12, 2006 will not be able to be delivered by December 23, the last date for mail delivery before Christmas.

The price for a set of Blocks in a Box™ is $99.00. There is an additional $10 fee for shipping and handling of each set. This fee is $15.00 for shipments to Canada.

I wish to order the optional custom-engraved bright brass name plate for an additional $5.00. The engraving can have up to three lines with up to 26 letters (including spaces) on each line. The text on each line will be centered and appear exactly as you enter it in the form below.
